need to accomplish to get there?”
Are you fresh out of school and wondering what to do now?
Have you been in a day program and now want to go to work?
We can help you make an employment plan that can help you
decide what you want to do in the future, what is realistic
for you and how you can get there.
Sometimes it may take a while to get to your goal but we
will try to make a path that will be just right for you.
Qualifications
Must be referred by Department of Developmental Disabilities
and over 21 years old .
Concerned Citizens
began in 1979 as a parent group concerned with providing
services to children with disabilities. It has become a
Social Service agency dedicated to quality care for not only
children and adults with disabilities but with senior
citizens.
Staff
are carefully screened and trained in providing the highest
quality of service for each individual need. They are
required to have a food handler’s permit, First Aid , CPR
and AIDs training, a TB test, and training in Person
Centered planning modalities.
Supervisors
are trained in assessing individual need, crisis
intervention, mediation, developmental disabilities, and
behavior management.
Services
We will
want to find out more about you, like what you like to do
and what you do best. To figure that out, we will want to
develop a team around you of people whom you choose that
will help you think of the things you could do in job. This
team will be people who know you well and can help you to
gain a clear idea of what you want to do and how you can get
what you need and want?

What are
your fears?
At the
end of your planning time you will have an Action Plan that
will outline what you want to do and how you can make it
happen step by step. You will have chosen an employment
vender to work with you on developing the job skills and
actually finding the job that fits you.
As you
exit the Pathways program you are armed with a good plan and
some of the tools necessary to make your job search easier
and more successful. |
