1. How do I qualify for services?
Contact your local case manager to set up an interview,
they will then refer you to the program you are eligible.
2. Who will decide what is done for
me?
Your local case manager will determine what program you
will qualify for and what services you will need.
3. Will I have to Pay?
Their may be a participation fee for some clients, this
is decided during your interview with your case manger.
4. When will someone come?
After you have had your interview, you will be informed
of your options and the agencies that can provide services
to best meet you needs. Once you have chosen and agency, the
agency Program Director will do an interview with you and
assign you a worker.
5. What are the rules for Personal
care?
Personal care is done in your home based on a care plan
that will be determined by the interview between you and
your case manager.
6. Are staff screened?
All staff must pass a background check and receive 22
hours of Fundamentals of Care training and an ongoing 10
hours of training every year, they must obtain a First
aid/CPR training, and become a Nursing Registered Assistant
and a two step TB test.
7. Can someone take me shopping?
When your care plan is developed by your and your case
manager shopping could be a part of your care plan.
8. What if I only need some one for
a week or two?
Our agency is flexible to the client’s needs, if you are
only going to require care for a small amount of time we
strive to fill that need.
9. What if I don't like my staff
person?
If you wish to change a staff person for any reason you
need to contact the Program Director for the program you are
assigned to and another staff person can be assigned.
10. What areas does your agency
serve?
Concerned Citizens serves both Clallam and Jefferson
counties.
Personal Care:
1. Can someone take me to the movie/zoo/out to dinner?
NO, THAT CAN BE CONFLICT OF INTEREST OR MISTAKEN FOR
EXPLOITING A CLIENT
2. Can I pay the
worker for gas money to take me places?
NO, THIS CAN BE MISTAKEN FOR EXPLOITING WE ARE
PROFESSIONALS
THAT HAVE RULES ON TAKING MONEY FROM CLIENTS
3. Can my family
members be caregiver for me?
YES, IF THEY GO
THRU THE HIRING PROCESS OF CC AND PASS THE BACKGROUND
CLEARANCE CHECK, THEY ALSO MUST COMPLY WITH THE TRAININGS
REQUIRED.
4. Can I get
help only when I need it?
WHEN CONTRACTING
WITH CC, WE SET A SCHEDULE THAT MEETS YOUR NEEDS. OUR
EMPLOYEES ARE WORKING WITH OTHER CLIENTS, SO THE SCHEDULE IS
NECESSARY, BECAUSE OF THIS WE CAN NOT BE ON AN "ON CALL
STATUS"
5. Can I
terminate services with CC at any time?
IT IS YOUR RIGHT
TO TERMINATE SERVICES WITH US, IF POSSIBLE, A TWO WEEK
NOTICE WOULD BE NICE
6. Can I get
services private paid by me and my family?
YES, CC WOULD
PROVIDE A CONTRACT TO YOU AND WE WOULD DOCUMENT THE NEEDS,
DATES AND TIMES NEEDED FOR YOU ALONG WITH THE SERVICES YOU
NEED. YOU WILL BE BILL MONTHLY FOR THESE SERVICES.
Children Programs:
1. What do I need in order to enroll my child/children in
child care?
There is a packet
of enrollment papers for parents, they include but are not
limited to, immunization records, your child’s last check up
from your family physician and dentist, and emergency phone
numbers, a list of people who may pick your child/children
up from the center among information to help us better
understand how to best serve your child.
2. If I am concerned about my child’s development how do
I request a developmental screening?
You may contact
Concerned Citizens, the Lead Agency for Birth to Three
services and they will direct you to a Family Resources
Coordinator in your area, for both Clallam and Jefferson
Counties.
3. Does you child care center accept subsidized child
care?
Yes our child care
agency accepts all forms of payment.
4. Does your agency have child care in any other areas
outside of the Forks area?
No, we have on
child care center located in Forks.
5. What age do you accept children?
We serve children
from one month through twelve years of age.
6. Do you provide transportation for child care services?
We provide limited
transportation within the city limits to families who have
transportation issues, there may be a small fee for
transportation.
7. What type of activities do you provide on a daily
basis for children?
We provide a
variety of activities during your child’s stay at our
center, including but not limited to, art, reading, free
play, physical activities to break up the day, such as going
to our playground where children can swing, play games, such
as baseball, climbing and many other options, if the weather
does not allow outside play we use our large muscle room,
where children can tumble, play games such as red light,
green light, twister and many other organized play.
