Adult Family Home

Twenty-four hour living accommodation and support for six residents who are developmentally delayed.

Services and activities will be determined based upon a written assessment made by a qualified assessor,  most often, a case manager and obtained prior to the Resident’s admission to Connecting Links.  The assessment will address specific information regarding the applicant including recent medical history, care needs and preferences, current prescribed medications, medical diagnosis, significant known behaviors or symptoms, history of depression, anxiety, and mental illness if applicable, social, physical and emotional strengths and needs, functional abilities, and an evaluation of cognitive status, and activities preferences. The qualified assessor will complete a preliminary service plan that describes the needs for services and an initial plan as to how to meet the needs identified in the assessment.

A more specific negotiated care plan about how the Resident’s needs and preferences will be addressed will be completed within thirty days of admission. This care plan will be completed in consultation with the Resident, legal representative if applicable, the professionals involved with the Resident appropriate Facility staff, Resident’s DSHS case manager, and any other person the Resident wishes to include.  It must be agreed to and signed by the Resident and/ or the Resident’s legal representative.

Connecting Links will notify the Resident, DSHS case manager, and the Resident’s legal representative as soon as possible of any changes in the Resident’s condition that require a different level of service.

Services Included are:

    • Room to include space of storage of clothing, and a reasonable amount of personal possessions, and adequate lighting.  The resident may use his or her own personal belongings and furniture, subject to space considerations and the safety of others.  If the resident desires a bed, linens, blankets and a pillow and a lockable storage space for small items of personal property will be provided.
    • Room assignment is the right of the facility.
    • Phone access to a non-pay phone that affords privacy to the resident
    •  Immediate notification to family if the negotiated care plan is to be changed.
    • Three nutritious meals per day, snacks, and beverages. Special dietary needs will be accommodated and individual food preferences will be taken into consideration.
    • Supervision as indicated in the Care plan, 24 hour staffing
    • Basic needs such as assistance with bathing, eating, hygiene, hair and body care.
    • Medical needs such as Doctor visits, Emergency treatment, dental and eye appointments.
    • Medication and nursing services assistance with medications and medical needs as identified in the assessment and care plan.  Medications will be kept in locked storage.
    • Therapy needs such as visits to therapists, (physical, occupational, speech).
    • Mental Health needs met such as counseling or mental health appointments,
    • A minimum of four group or individual outings per month and or any planned activity programs listed in the negotiated care plan.
    • Common use items such as toilet paper, linen, soap, shampoo, and any non-personal items.
    • Laundry services will be provided as needed, sheets and pillowcases will be laundered weekly or more frequently as needed.
    • Housekeeping and cleaning services
    • Upkeep on physical aspect of home including repairs and replacement of furniture.
    • Professional hair cuts every two months at a minimum

B.     Services not covered by basic rate participation fee:

  • Social outings and activities that require a fee or money to realize.
  • Shopping for individual items specific to the residents use only deodorant, special shampoo, shaving cream, cologne, and lotions.
  • Trips and outings outside of a twenty mile radius of the Adult Family Home
  • Specialized shampoos or personal care items that are specific to the Resident.
  • Outings and activities beyond those provided by the Adult Family Home as listed in Basic Services Rate.
  • Medical and Dental visits outside Clallam County
  • Any care outside the realm of Nurse delegation will not be performed in the home.
  • Some services may be at an additional cost to the resident, such as on site health care services requested by the resident or as ordered by the resident’s physician.
  • Beauty shop services that are above and beyond haircuts and general hair maintenance, such a perms, styling, nail care, coloring.

Accommodations to Resident Care:

It is our intent to provide the best care possible to our residents.  Residents needs change and we expect needs to go up and down, we will accommodate that with changes in staffing and innovative problem solving to provide the best possible environment for each clients individual needs.  Connecting Links is willing to make necessary adjustments to accommodate a resident’s increased care needs to the greatest extent possible in order to maximize quality of life for the individual as long as that care will not put the resident at risk.

Staff:

All caregivers employed by the Adult Family Home have successfully passed a background clearance, has a current First Aid card, HIV/AIDs training, and a two-step TB test.  Each caregiver has completed Fundamentals of Care giving, is a Nursing Assistant Registered and has had Nurse Delegation training.
