Home Care


HOME CARE PROGRAMS
FOR
CHILDREN, ADULTS, AND SENIOR CITIZENS WITH DISABILITIES.

When your loved one cannot live alone it is hard to know what to do. Home Care is one option that allows them to live as independently as possible with assistance and stay at home.

We can help you find solutions that will work for your family and help you plan ahead.

Finding the level of care and resources you need to provide the best possible quality of life may be a hard decision.

 We can help!

If you don’t qualify for any of the State programs we may be able to custom fit a program that you can afford.

Don’t be afraid to ask!

 

Creating Connections is the Home Care program of Concerned Citizens and provides programs for Medicaid Personal Care, COPES, and CHORE.

Staff are carefully screened and trained in providing the highest quality of care for each individual need. They are required to have First Aid, CPR and AIDS training, a TB test, Fundamentals of Care giving and 10 hours of continuing education each year.  Staff are (NAR), Nursing Assistant Registered.

Supervisors try hard to maintain consistency, knowing that it is important to the client and quality care. They are trained in assessments, mediation, and crisis intervention.

Linda Middleton: the Executive Director is a Licensed Practical Nurse, Early Childhood Educator, and has had Specialty Training for Developmental Disabilities. She is also a certified adult educator.

Provides services to clients that allow them to live independently in their home and community.  Clients can interview with staff to help choose a compatible worker.

Transportation

  • Transportation to shopping.

  • Transportation to Medical and Dental appointments.

  • Transportation to Physical Therapy appointments.

  • Transportation to Occupational Therapy appointments.

  • Transportation to Speech therapy appointments.

Transportation is limited
 

Payment Options and Fees

Many clients qualify for help through the state that pays all or part of their care.  We can help you get to the appropriate agency to find out if you qualify for COPES, CHORE, PERSONAL CARE, OR RESPITE.

Private Pay charges are $15.00 per hour for a minimum of 2 hours per visit.
